Fill in the document at the bottom of the page. The document refers to your personal details and office policies. The documents need to be filled in , signed and send before the session to the following email: firstname.lastname@example.org
Call or email for availability
Pre-pay the amount of the session at least 1 day before the scheduled appointment. The invoice will be sent via e-mail after the session.
Fee & Payment Agreement
Psychotherapy fee structure is based on a 60-minute hour and costs 85euros incl. VAT/BTW. Conjoint and family therapy sessions are typically scheduled for 90 minutes and costs 121euros incl. VAT/BTW. The fee is pre-paid 1 day before the actual appointment by deposit in following bank account.
Bank account details: NL50 ABNA 0454069464. Failure to do so the appointment will be cancelled.
SKYPE session fees: 75euros/hour incl. VAT/BTW.
Payment is due within 14 days of the invoice date, without any deduction, discount or set-off, by depositing or transferring the payable amount to the bank account stipulated by the user. Objections to the level of the bills do not suspend the payment obligation.
If the patient fails to remit payment within the 14-day period, the patient shall be held in default by operation of law. The patient shall in that case be liable for the payment of interest equal to the statutory commercial interest rate at that time plus any other debt collection and legal costs occurs. The interest over the payable amount shall be calculated from the time at which the client was held in default until the time of full and final settlement, in which context part of a month shall be deemed to be a full month.
In the event of the patient being liquidated, declared bankrupt or granted suspension of payment, the claims of the user on the patient shall become immediately due and payable.
Check with your healthcare insurance
As the counselor is not BIG registered, we recommend that you consult your health insurance company regarding therapy refunds. Different insurance companies have different refund policies. Some offer a full refund or a maximum number of sessions. Unfortunately no refunds are granted in some cases.
Cancellation of changes of sessions can be done 24 hours in advance. The user is authorized to charge a cancellation fee of 85euros (incl. VAT) if patient fails to cancel the session 24 hours in advance.
Policy regarding canceling appointments was developed to protect both therapist and client. When you schedule an appointment with the therapist, a time slot in his schedule is being reserved for you. Because it can be difficult to fill a time slot when an appointment has been canceled at last minute, you will be expected to provide at least 48 hours’ notice or you will be charged for the session time that had been reserved for you. In the case of sudden illness or an emergency that makes it impossible to provide 48 hours’ notice, the therapist will make a reasonable effort to reschedule the appointment for a later time within the next few days. In such cases, you will be billed only for the rescheduled appointment. The 48-hour notice requirement does not apply to the initial “intake” interview, to appointments that have been reserved for psychological assessment, or to instances in which the canceled meeting was to have been the second therapy session of that week.
To facilitate the openness that is a key facet of the psychotherapy relationship, the client must be able to trust that the information shared in the therapy session will be held in the strictest confidence. As such, M.Stella is ethically bound to protect the confidentiality of her patients. However, it is important to clarify that there are legal and ethical limits to the patient’s right to confidentiality. The therapist is compelled to breach confidentiality in cases in which a patient appears to present a physical danger to himself or to another person or in which a child or elderly adult has been abused or neglected.
